چکیده
Abstract Background/Aims Hydroxychloroquine (HCQ) is used widely in the management of rheumatic diseases and considered to be generally well tolerated. HCQ ocular toxicity a dose- time-related reaction. The Royal College Ophthalmologists (RCOphth) noted low rate drug discontinuation as result baseline testing (< 4%) that prevalence data for was based on treatment duration >5 years. RCOphth updated their screening guideline 2020. A major change removal requirement screening, with commenced at 5 years unless risk factors are present. Though number patients starting UK prescribing could estimated, proportion remaining stated unknown. We reviewed departmental prescribing, adverse event attrition over from Bolton NHS Foundation Trust, district general hospital which serves population nearly 300,000. Recognising adverse events and critical incidents in medical practice in a district general hospital.
A pilot audit of case records of consecutively discharged patients from a district general hospital was undertaken by specialist registrars, SHOs and senior nurses in order to identify adverse events (AEs) and critical incidents (CIs) related to hospital care. Experienced external assessors taught the clinical staff to use a previously validated structured method of case record review that faci...

The increase of postoperative stay due to complications may be reduced by lowering the complication rate and, in the l...
متن کاملChoosing appropriate theories for understanding hospital reporting of adverse drug events, a theoretical domains framework approach
Adverse drug events (ADEs) may cause serious injuries including death. Spontaneous reporting of ADEs plays a great role in detection and prevention of them, however, underreporting always exists. Although several interventions have been utilized to solve this problem, they are mainly based on experience and the rationale for choosing them has no theoretical base. The vast variety of behavioral ...

In a 12-month prospective survey of CPR (cardiopulmonary resuscitation), 32 out of 192 patients (16.6%) survived to go home. This is a clear improvement compared with 7 years previously. This is attributed to better training in the use and management of CPR and more widespread availability of defibrillators. Certain patients could not be resuscitated--those with electromechanical dissociation, ...
